Why the Buy and Sell are happening at the same level?

Guys, can you help? Why my buy and sell are happening at the same level?
I´ve put some conditions and when the conditions is reached I open a buy or sell with take profit and stop loss. But, the buy and the sell are happening at the same time and same level.
