EA opens trades at max lot size

My EA sometimes opens trades at the max lot size, this never occurs in the strategy tester. SL & TP are correctly placed, but what's supposed to be 0.4% risk ends up being 50 lots (like the GBPJPY trade in the attached pic).

How big is your balance? And how far is the SL from current price? The logic is correct.
